Updated physicals required for incoming freshman and juniors, dated after May 1st  of students' eighth and tenth grade years. Completed physicals must be uploaded to students' big teams account prior to the first practice.

Interim Questionnaire

Interim questionnaire required for incoming sophomores and seniors (with current physical on file), dated after May 1st  of students' freshman and junior years. Completed questionnaire must be uploaded to students' big teams account prior to the first practice.

Travel Release

Travel Release needs signed by athletic department following parent guardian signature, students return signed form to their coach prior to departure for event.
