Physical Form

Access the IHSAA sports physical form here. This form is to be completed for all Freshmen and Juniors. Sophomores or Seniors who are participating for the first time also need to have a current sports physical. These forms need to be completed after May 1st to be valid for the upcoming school year (24-25).
